    Summary

Diagnosing brain arteriovenous malformations is challenging. These rare vascular brain lesions can appear as non-enhancing masses on CT and angiography, mimicking other brain tumors.

    Area of Science:

    • Neurology
    • Neuroradiology
    • Neuro-oncology

    Background:

    • Arteriovenous malformations (AVMs) in the brain are uncommon vascular anomalies.
    • Angiographically occult AVMs present significant diagnostic challenges in clinical practice.

    Observation:

    • Four cases of angiographically occult AVMs are presented.
    • Patients presented with epilepsy and focal neurological deficits in early adulthood.

    Findings:

    • CT scans revealed hypodense lesions with minimal or no contrast enhancement.
    • Angiography demonstrated only an avascular mass, leading to misdiagnosis.
    • Differential diagnoses included gliomas, dermoid tumors, and cerebral infarcts prior to histological confirmation.

    Implications:

    • Highlights the difficulty in diagnosing certain brain AVMs.
    • Emphasizes the need for high suspicion and advanced imaging in cases with atypical presentations.
    • Underscores the importance of histological examination for definitive diagnosis of suspected brain lesions.
