    Summary

    Glomerular foam cells, typically seen in nephrotic syndrome, were identified in a kidney transplant recipient. The cause of these lipid-laden mesangial cells in the allograft remains unclear.

    Background:

    • Foam cells in glomeruli are characteristic of nephrotic syndrome.
    • Their presence in renal allografts has not been previously documented.

    Observation:

    • Glomerular foam cells were identified in a cadaveric kidney transplant.
    • The patient was a 35-year-old male with a functioning allograft for 7 months.

    Findings:

    • Electron microscopy confirmed mesangial cells heavily laden with lipids.
    • This represents the first reported instance of glomerular foam cells in a renal allograft.

    Implications:

    • The findings suggest a potential, previously unrecognized complication in renal transplantation.
    • Further research is needed to elucidate the etiology of foam cell formation in allografts, potentially related to hyperlipidemia or other factors.
