Background:

Chest wall lesions can present diagnostic challenges on mammography.
Accurate interpretation is crucial for patient management.

Observation:

A 67-year-old patient presented with right axillary and supraclavicular fullness.
Mammography revealed findings suggestive of a chest wall mass.

Findings:

Imaging, including mammography, ultrasonography, and CT angiography, diagnosed an axillary and retro-pectoral vascular malformation.
This vascular anomaly mimicked a malignant mass on initial mammographic assessment.

Implications:

This case underscores the importance of considering vascular malformations in the differential diagnosis of mammographic abnormalities.
Advanced imaging modalities are essential for definitive characterization and diagnosis of chest wall lesions.
