Summary

Magnetic resonance-guided focused ultrasound is a new incisionless treatment for essential tremor (ET). This study shows it alters brain networks, reducing cerebellar excitatory input to the sensorimotor cortex.

Background:

  • Essential tremor (ET) is a neurological condition.
  • Magnetic resonance-guided focused ultrasound (MRgFUS) offers a novel, incisionless ablative treatment targeting the ventral intermediate nucleus for ET.

Purpose of the Study:

  • To investigate the structural and functional network alterations following unilateral MRgFUS sonication of the ventral intermediate nucleus in patients with ET.

Main Methods:

  • Fifteen ET patients underwent diffusion tensor imaging (DTI) and functional MRI (fMRI) during tremor-eliciting tasks.
  • Scans were performed pre-procedure, and at 1 and 6 months post-unilateral sonication.
  • Probabilistic tractography and dynamic causal modeling (DCM) were employed to analyze network changes.

Main Results:

  • Tractography revealed alterations within the dentato-thalamo-cortical tract (DTCT) post-sonication.
  • Reduced task-evoked activation was observed in the contralateral sensorimotor cortex and ipsilateral cerebellar regions.
  • DCM indicated a significant decrease in excitatory drive from the cerebellum to the contralateral sensorimotor cortex.

Conclusions:

  • Unilateral sonication of the ventral intermediate nucleus induces specific functional network changes within the DTCT.
  • These changes include a reduction in excitatory cerebellar input to the sensorimotor cortex in ET patients.
