Summary

Giant cell arteritis can cause sudden hearing loss in older adults. Prompt corticosteroid treatment can significantly improve hearing, even with mixed deafness.

Background:

  • Giant cell arteritis (GCA) is an inflammatory condition primarily affecting large arteries.
  • Hearing loss is an uncommon but possible manifestation of GCA.

Observation:

  • A 75-year-old woman presented with GCA symptoms and bilateral mixed hearing loss.
  • Audiometry confirmed the hearing impairment, which included both sensorineural and conductive components.

Findings:

  • Treatment with corticosteroids led to rapid clinical and biological improvement.
  • Follow-up audiometry at three months showed significant improvement, with reduced sensorineural loss and resolved conductive hearing loss.

Implications:

  • Sudden onset hearing loss in elderly patients warrants investigation for GCA, especially in an inflammatory context.
  • Diagnosis can be challenging due to atypical presentations and negative temporal artery biopsies.
  • Corticosteroids are generally effective in managing GCA-associated hearing loss.
