    Background:

    • Advanced cervical cancer (N3) with large, fixed nodes presents treatment challenges.
    • Conventional radiotherapy schedules can be lengthy, impacting patient compliance and resource utilization.

    Purpose of the Study:

    • To evaluate the efficacy and safety of an accelerated irradiation technique combining electron beam and cobalt sources.
    • To assess the feasibility of reducing treatment duration while maintaining therapeutic radiation doses.

    Main Methods:

    • A concomitant boost technique was employed in 34 N3 cervical cancer patients.
    • Treatment combined conventional cobalt irradiation with electron beam boosts over 5 weeks, delivering 70 Gy to enlarged nodes.
    • The primary tumor site also received a 70 Gy boost when identified.

    Main Results:

    • The accelerated schedule reduced overall treatment duration from 7 to 5 weeks.
    • Acute skin reactions were acceptable, with no major mucosal complications reported.
    • Complete clinical regression was observed in 62% of patients post-irradiation, though 41% experienced subsequent cervical recurrences.

    • The concomitant boost technique effectively accelerates irradiation for N3 cervical cancer.
    • This method maintains the total tumor dose and does not increase immediate complication rates.
    • Further investigation is warranted to address the observed rate of cervical recurrences.
