    Summary

A significant portion of men experiencing erectile dysfunction have abnormal glucose metabolism, including newly diagnosed diabetes. This highlights the importance of screening for metabolic disorders in men with impotence.

    Area of Science:

    • Endocrinology
    • Urology
    • Men's Health

    Background:

    • Erectile dysfunction (ED) is a common concern affecting many men.
    • The underlying causes of ED can be multifactorial, including physiological and psychological elements.

    Purpose of the Study:

    • To investigate the prevalence of abnormal glucose metabolism in men presenting with erectile dysfunction.
    • To determine the association between glucose metabolism abnormalities and the type of erectile dysfunction (organic vs. psychogenic).

    Main Methods:

    • Evaluation of 497 men with erectile dysfunction over 14 months.
    • Initial assessments included medical history, physical examination, and laboratory tests (serum testosterone, prolactin, nocturnal penile tumescence).
    • Further investigations comprised penile vascular studies, oral glucose tolerance tests, and psychiatric consultations as needed.

    Main Results:

    • Abnormal glucose metabolism was identified in 32% of the evaluated men.
    • This included newly diagnosed noninsulin-dependent diabetes mellitus (11.1%) and impaired glucose tolerance (4.2%).
    • Organic pattern impotence was diagnosed in 91.3% of men with abnormal glucose metabolism.

    Conclusions:

    • Abnormal glucose metabolism, particularly diabetes mellitus, is highly prevalent in men with erectile dysfunction.
    • Screening for glucose metabolism disorders is crucial in the comprehensive evaluation of men with ED.
    • Metabolic health significantly impacts erectile function, with a strong link to organic impotence.
