    Background:

    • Glaucoma filtering surgery is a common procedure to reduce intraocular pressure.
    • Complications can arise, necessitating careful patient selection and surgical technique.

    Observation:

    • A review of 305 glaucoma filtering procedures identified five cases (1.6%) of postoperative suprachoroidal hemorrhage.
    • All affected eyes were aphakic, with four having a prior partial or complete vitrectomy.

    Findings:

    • The incidence of suprachoroidal hemorrhage was significantly higher in aphakic eyes (13%) and even more so in aphakic, vitrectomized eyes (33%).
    • Despite intervention, three patients experienced poor visual outcomes.

    Implications:

    • Aphakia and prior vitrectomy are critical risk factors for suprachoroidal hemorrhage during glaucoma surgery.
    • Surgeons should exercise caution and consider alternative strategies for high-risk patients.
