Background:

  • The COVID-19 pandemic underscored the need for tools to prioritize high-risk patients for urgent evaluation.
  • Gross hematuria is a common presenting symptom requiring prompt assessment for underlying malignancy.

Purpose of the Study:

  • To evaluate the Glasgow Prognostic Score (GPS) as a predictor of high-grade and high-stage urothelial bladder cancer.
  • To determine if GPS can identify patients with gross hematuria who need urgent evaluation.

Main Methods:

  • A retrospective analysis of 101 patients presenting with gross hematuria was conducted.
  • Glasgow Prognostic Score (GPS) was calculated using pretreatment C-reactive protein (CRP) and albumin levels.
  • Multivariate logistic regression analyzed the relationship between GPS and final bladder cancer diagnosis.

Main Results:

  • A GPS of 1 or 2 was significantly associated with high-grade tumors (67% vs. 13%, p<0.0001).
  • A GPS of 2 predicted muscle-invasive disease (48% vs. 6%, p=0.04).
  • Multivariate analysis confirmed GPS as an independent predictor of high-grade and high-stage bladder cancer.

Conclusions:

  • The Glasgow Prognostic Score (GPS) is a readily accessible predictor of high-grade and high-stage urothelial bladder tumors.
  • GPS can assist in identifying patients with gross hematuria who require urgent urological evaluation.
