Background:

  • Understanding the predictors and correlates of PSF is crucial for effective management.

Purpose of the Study:

  • To explore features predicting long-term post-stroke fatigue (PSF) after ischemic stroke.
  • To investigate cross-sectional correlations between PSF and various health outcomes 7 years post-stroke.

Main Methods:

  • Prospective cohort study (SAHLSIS) with 430 participants (18-69 years at stroke).
  • Baseline data included stroke severity and cardiovascular risk factors.
  • PSF, cognitive, neurological, functional outcomes, depression, anxiety, pain, insomnia, and physical activity assessed at 7 years post-stroke using D-FIS and other validated scales.
  • Ordinal regression and correlation analyses (Spearman's, point-biserial) used for associations, including sex-stratified analyses.

Main Results:

  • 80% of participants reported significant fatigue 7 years post-stroke.
  • Female sex and higher stroke severity were independently associated with PSF.
  • PSF correlated strongly with poorer functional, neurological, and cognitive outcomes, plus depressive symptoms, anxiety, insomnia, pain, and reduced physical activity (p < .001).
  • Insomnia showed a stronger correlation with PSF in women than in men (p = .03).

Conclusions:

  • PSF is highly prevalent and persistent in the long term after ischemic stroke.
  • A complex interplay exists between PSF, sex, and multiple functional and psychological outcomes.
  • Further research is needed to establish causal relationships and develop targeted interventions for PSF and associated conditions.
