Background:

  • Major depressive disorder (MDD) is a prevalent mental disorder linked to brain activity alterations, particularly in prefrontal regions, insula, anterior cingulate cortex, and amygdala.
  • Existing research on MDD brain activity is often heterogeneous, potentially due to overlooking comorbid anxiety (COM-A).

Purpose of the Study:

  • To investigate neurofunctional differences in brain activity during emotional processing between patients with MDD and those with MDD comorbid with anxiety (COM-A).
  • To explore the relationship between these brain activity patterns and measures of anxiety, depression, general functioning, and medication load.

Main Methods:

  • Functional MRI (fMRI) was used to compare brain activity in 90 MDD patients and 85 COM-A patients during a negative emotional face-matching task.
  • A hypothesis-driven region of interest approach (dorsolateral prefrontal cortex, insula, anterior cingulate cortex, amygdala) and a whole-brain approach were employed.
  • State-trait anxiety, depression, general functioning, and medication load were also assessed.

Main Results:

  • COM-A patients exhibited significantly increased bilateral insula activity compared to MDD patients during face processing.
  • No significant differences in activity were observed in the anterior cingulate cortex or amygdala between the groups.
  • Whole-brain analyses revealed increased inferior temporal and frontal activation in COM-A, positively correlated with state anxiety and general functioning.

Conclusions:

  • The study identified distinct activation patterns in the insula differentiating MDD from COM-A during emotion processing.
  • These findings suggest a potential neural correlate for anxiety-related psychopathology in depression.
  • Further research is needed to validate these results and understand the implications for the origin, development, and treatment of depression.
