Summary

Laparoscopic surgery for large simple renal cysts (SRCs) improved kidney tissue thickness but did not significantly enhance renal function. Observation may be a safe alternative for large SRCs regarding kidney function.

Background:

  • This study investigates the impact of laparoscopic cyst unroofing on renal function in patients with large SRCs.

Purpose of the Study:

  • To evaluate the effect of laparoscopic simple renal cyst (SRC) surgery on renal function.
  • To assess changes in plasma creatinine, estimated glomerular filtration rate (GFR), and 24-hour urinary parameters post-surgery.
  • To determine if improvements in renal parenchymal thickness correlate with changes in renal function.

Main Methods:

  • A case-series study involving 22 patients with SRCs larger than 50 mm.
  • Laparoscopic cyst unroofing surgery was performed.
  • Renal function was assessed by measuring plasma creatinine, estimated GFR, and 24-hour urinary protein, creatinine, and volume before and 1 month after surgery.
  • Renal parenchymal thickness was measured using CT scans before and after surgery.

Main Results:

  • No significant differences were observed in plasma creatinine or estimated GFR before and after surgery.
  • 24-hour urinary volume, creatinine, and protein showed no significant changes post-surgery.
  • A statistically significant improvement in renal parenchymal thickness was noted after surgery (10.4 ± 0.7 mm, P=0.001).
  • The improvement in parenchymal thickness did not correlate with changes in estimated GFR (r=0.349, P=0.13).

Conclusions:

  • Laparoscopic unroofing of large simple renal cysts can lead to noticeable recovery of renal parenchymal thickness.
  • The surgery did not result in significant improvements in overall renal function.
  • Observation may be a safe and viable treatment strategy for large SRCs, without compromising renal function.
