Summary

Lymphovascular invasion (LVI) in prostatectomy specimens is often underreported. Improved identification criteria and awareness of LVI morphology can enhance diagnostic accuracy and predict bone metastasis risk.

Background:

  • Lymphovascular invasion (LVI) in radical prostatectomy (RP) specimens is linked to higher metastatic disease risk.
  • Variable reporting rates suggest potential underdiagnosis of LVI.

Purpose of the Study:

  • To characterize LVI foci in RP specimens, including location, quantity, and morphology.
  • To assess the association between LVI and cribriform/intraductal carcinoma and metastatic risk.

Main Methods:

  • Retrospective review of 54 RP specimens initially reported as LVI-positive.
  • Confirmation of ambiguous cases using ERG, CD31, and D2-40 immunostaining.
  • Analysis of LVI characteristics and correlation with clinical outcomes.

Main Results:

  • LVI was not confirmed in 7.4% of cases; common mimickers included retraction artifact and dislodged cells.
  • Key diagnostic criteria for LVI included corrugated vascular lining, endothelial bulging, and proteinaceous material.
  • LVI was frequently associated with cribriform and/or intraductal carcinoma (94%).
  • Higher LVI frequency independently predicted increased risk of bone metastases (P=.009).

Conclusions:

  • Detailed characterization of LVI morphology and location can improve diagnostic accuracy in prostatectomy pathology.
  • Awareness of LVI features aids in distinguishing true invasion from mimickers.
  • Quantification of LVI may be clinically significant for predicting prostate cancer metastasis, warranting further validation.
