    Summary

This case study reveals that reading epilepsy seizures were triggered only by Spanish text, not English. This supports the communication hypothesis over other stimuli theories for reading-induced seizures.

    Area of Science:

    • Neurology
    • Epileptology
    • Neurophysiology

    Background:

    • Reading epilepsy is a rare form of reflex epilepsy.
    • Seizure triggers in reading epilepsy are diverse, including visual stimuli and cognitive processing.
    • The underlying mechanisms remain debated, with various hypotheses proposed.

    Observation:

    • A 21-year-old woman with a history of reading epilepsy was studied.
    • Electroencephalographic (EEG) and clinical observations were recorded during various reading and cognitive tasks.
    • Stimuli included reading Spanish and English magazines, repetitive announcement reading, viewing silent comic strips, and mathematical calculations.

    Findings:

    • Seizures and EEG abnormalities (paroxysms) were exclusively observed during reading in Spanish.
    • No seizures were elicited by reading in English, repetitive reading of announcements, viewing comic strips, or performing calculations.
    • This specific response pattern suggests language processing is a key factor.

    Implications:

    • The findings support the 'communication' hypothesis of reading epilepsy.
    • This challenges theories focusing solely on visual or proprioceptive stimuli.
    • Further research into language-specific triggers in reflex epilepsies is warranted.
