    Summary

    Asthma symptoms can appear before airway hyperresponsiveness develops. This study observed patients whose asthma symptoms preceded measurable hyperresponsiveness to carbachol, suggesting it's not always a prerequisite.

    Background:

    • Asthma is typically characterized by airway hyperresponsiveness to bronchoconstrictors.
    • However, the temporal relationship between asthma symptoms and hyperresponsiveness is not fully understood.

    Purpose of the Study:

    • To investigate whether asthma symptoms can manifest in the absence of airway hyperresponsiveness.
    • To determine if airway hyperresponsiveness can develop after the onset of asthma symptoms.

    Main Methods:

    • Case study of three patients presenting with asthma symptoms but initially lacking hyperresponsiveness.
    • Assessment of airway responsiveness using specific airway resistance (SRaw) measurement after carbachol challenge.
    • Repeated carbachol challenge after 6-18 months to track changes in responsiveness.

    Main Results:

    • Initially, patients showed no significant change in SRaw after carbachol inhalation.
    • On follow-up, carbachol responsiveness increased into the asthmatic range.
    • Baseline SRaw values remained unchanged, and the bronchodilator effect of maximal inspiration was ruled out as an explanation.

    Conclusions:

    • Airway hyperresponsiveness may not be a necessary condition for the initial development of asthma.
    • Symptoms of asthma can precede the measurable appearance of airway hyperresponsiveness.
