Primary Healthcare Services

1.5K
Primary care promotes wellness and prevents disease. This care includes health promotion, education, protection (such as immunizations), early disease screening, and environmental considerations. Settings providing this type of healthcare include physician offices, public health clinics, school nursing, and community health nursing.

Planning Nursing Care I

4.7K
The planning phase of the nursing process helps nurses set priorities, outline patient-centered goals and expected outcomes, and tailor nursing interventions to align with the aligned care plan. Through the planning phase, the nurse applies critical thinking skills to align and develop interventions according to the patient's needs. It provides continuity of care allowing patients to receive the maximum benefit from treatment. It serves as a pilot plan for allocating individual staff to a...

Rural Health Centers
Rural health centers are specialized care facilities in remote locations with very few medical personnel. The primary care providers who run the centers are mostly Registered Nurse Practitioners. Here, emergency treatment is provided to critically ill or injured patients before they are transferred to the closest hospital. Fortunately, due to advancement in technology, many rural healthcare facilities and professionals have easy access to diagnostic and treatment...

Methods Of Healthcare Delivery System

3.5K
At the different levels of the healthcare system, we see varying methods of healthcare used. These methods include managed care systems, case management, and primary healthcare.
Managed Care System:
The managed care system is designed to control the cost while maintaining the quality of care. The patient's care from admission to discharge is planned by the primary care provider or the case manager, also known as the gatekeeper. In a managed care system, the number of care providers is...

Summary

Postpartum patients at federally qualified health centers have low primary care transition rates. Losing insurance significantly reduces the odds of accessing primary care, highlighting a barrier to maternal health continuity.

Area of Science:

  • Health Services Research
  • Maternal Health
  • Primary Care Transition

Background:

  • Transitioning from postpartum care to primary care is recommended for improved maternal outcomes.
  • Real-world patterns of this transition remain understudied, particularly in diverse healthcare settings.

Purpose of the Study:

  • To describe transition patterns to primary care for postpartum patients.
  • To identify predictors of this transition in a cohort from federally qualified health centers.

Main Methods:

  • Analysis of electronic health record data (2017-2019) for 7,926 postpartum patients.
  • Examined predictors including insurance loss, prior visits, and high-risk status.
  • Primary outcome: completion of a primary care visit within 6 months postpartum.

Main Results:

  • Only 17.3% of the full cohort and 25.0% of high-risk patients completed a primary care visit.
  • Insurance loss was a significant barrier, reducing odds of primary care access by 66%.
  • Several pre-pregnancy and postpartum visit factors predicted transition success.

Conclusions:

  • Federally qualified health centers exhibit low postpartum to primary care transition rates.
  • Insurance loss is a critical barrier that warrants targeted interventions.
  • Future research should address structural inequities and the impact of primary care on maternal health.
