Summary

Placentas from in vitro fertilization (IVF) conceived monochorionic diamniotic (MCDA) twins show no significant differences in placental sharing, angioarchitecture, or birth weight discordance compared to spontaneously conceived twins. These findings suggest outcomes may not stem from placental variations.

Background:

  • Monochorionic diamniotic (MCDA) twin pregnancies are high-risk.
  • In vitro fertilization (IVF) is increasingly used for conception.
  • Placental characteristics in IVF-conceived MCDA twins are largely unknown.

Purpose of the Study:

  • To compare placental characteristics between MCDA twins conceived via IVF and spontaneous conception.
  • To investigate differences in placental sharing, angioarchitecture, and cord insertion.

Main Methods:

  • Retrospective analysis of placental data from MCDA twin pregnancies.
  • Comparison of placental sharing, birth weight discordance, angioarchitecture, and umbilical cord insertion types.
  • Statistical analysis of data from 256 MCDA placentas (32 IVF, 224 spontaneous).

Main Results:

  • No significant differences were found in placental sharing, birth weight discordance, or angioarchitecture between IVF and spontaneous MCDA twins.
  • A trend towards more discordant umbilical cord insertions was observed in IVF-conceived twins (25% vs. 12%).

Conclusions:

  • MCDA placentation does not differ significantly between spontaneous and IVF conception.
  • Observed differences in pregnancy outcomes between spontaneous and IVF-conceived MCDA twins are unlikely to be due to placental sharing, angioarchitecture, or cord insertion type.
