Summary

Primary central nervous system lymphoma (PCNSL) patients with ocular involvement showed similar outcomes to those with CNS involvement. Initial tumor location did not impact treatment response or overall survival in this study of PCNSL progression.

Background:

  • Primary central nervous system lymphoma (PCNSL) is a rare non-Hodgkin lymphoma.
  • Understanding inter-compartmental progression and treatment outcomes in PCNSL is limited.
  • This study investigates outcomes based on initial site of PCNSL involvement.

Purpose of the Study:

  • To compare treatment outcomes and progression patterns in patients with ocular versus CNS primary central nervous system lymphoma (PCNSL).
  • To evaluate the influence of initial compartment of involvement on response rates, progression-free survival (PFS), and overall survival (OS).

Main Methods:

  • Multicenter retrospective cohort study of 234 PCNSL patients (2000-2018).
  • Patients categorized into ocular (Group 1) and CNS (Group 2) involvement groups.
  • Analysis of treatment modalities, complete response rates, PFS, and OS.

Main Results:

  • Complete response rates were 43% for ocular and 48% for CNS PCNSL.
  • Two-year PFS was significantly higher for CNS (56%) vs. ocular (35%) PCNSL (p < 0.0001).
  • Age under 60 was associated with longer PFS; initial compartment did not affect OS.

Conclusions:

  • Initial site of involvement in primary central nervous system lymphoma (PCNSL) does not significantly influence local response rates or overall survival.
  • While PFS differs, ocular and CNS PCNSL share similar long-term survival trajectories.
  • Age is a significant prognostic factor for progression-free survival in PCNSL.
