Purpose of the Study:

  • To conduct a meta-analysis assessing the efficacy of absorbable suture versus metal staples for cesarean skin closure.
  • To determine if suture remains the recommended method, irrespective of patient obesity status.
  • To analyze wound complication rates associated with different closure methods in cesarean deliveries.

Main Methods:

  • A systematic search of MEDLINE, PubMed, Cochrane Database, and ClinicalTrials.gov was performed.
  • Included were randomized controlled trials (RCTs) comparing absorbable suture with metal staples for cesarean skin closure.
  • Fourteen studies were included in the meta-analysis, with data abstracted and analyzed using fixed and random effects models.

Main Results:

  • Suture closure significantly decreased overall wound complications by 50% compared to staples (RR 0.59).
  • Wound separation was significantly reduced with suture closure (RR 0.43).
  • These benefits were consistent in patients with obesity, showing a significant decrease in wound complications with suture (RR 0.51).

Conclusions:

  • Absorbable suture closure of cesarean skin incisions is superior to metal staples in reducing wound complications.
  • The advantage of suture closure is maintained regardless of patient obesity status.
  • Suture closure remains the recommended method for cesarean skin closure.
