Intermittent parathyroid hormone-related peptide (PTHrP) treatment improves temporomandibular joint osteoarthritis (TMJ-OA) by enhancing bone formation and suppressing TGFβ signaling. This suggests PTHrP may be a novel therapeutic for TMJ-OA.
Area of Science:
- Biochemistry
- Cell Biology
- Orthopedics
Background:
- Temporomandibular joint osteoarthritis (TMJ-OA) involves complex signaling pathways.
- The roles of parathyroid hormone-related peptide (PTHrP) and transforming growth factor-β (TGFβ) in TMJ-OA pathogenesis are not fully understood.
- Disordered occlusion is a common cause of TMJ-OA.
Purpose of the Study:
- To investigate the therapeutic potential of PTHrP in a TMJ-OA animal model.
- To elucidate the underlying molecular mechanisms of PTHrP action in TMJ-OA, focusing on bone remodeling and TGFβ signaling.
Main Methods:
- Established a TMJ-OA animal model using disordered occlusion.
- Administered intermittent PTHrP (iPTH) to model animals.
- Utilized immunohistochemistry, histology, Micro-CT, histomorphometry, Western Blot, and in vitro cell culture (mesenchymal stem cells) to analyze condylar changes, bone formation, and signaling pathways.
Main Results:
- iPTH treatment significantly reduced condylar cartilage degeneration in TMJ-OA model animals.
- iPTH promoted subchondral bone formation by increasing osteoblast differentiation (OSX, OCN positive cells) and osteoclast numbers, leading to increased bone volume.
- In vitro, PTHrP enhanced osteoblastic differentiation of mesenchymal stem cells and inhibited TGFβ signaling, while PTHrP-PTH1R signaling suppressed TGFβ signaling during osteoblastic differentiation.
Conclusions:
- Intermittent PTHrP administration is effective in improving TMJ-OA lesions by enhancing subchondral bone osteoblastic differentiation.
- PTHrP exerts its therapeutic effects, in part, by suppressing aberrant TGFβ signaling.
- PTHrP targeting the TGFβ pathway represents a promising biological reagent for preventing and treating TMJ-OA.

Fractures: Bone Repair
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the...
