Summary

Combining L1-regularized iterative SENSE reconstruction with echo planar imaging (EPI) in hepatic diffusion-weighted imaging (DWI) can accelerate scans or improve resolution. However, this method may reduce the conspicuity of focal liver lesions, warranting further investigation.

Purpose of the Study:

  • To evaluate the utility of combining L1-regularized iterative SENSE reconstruction with single-shot EPI for hepatic DWI.
  • To compare this novel technique (L1-DWI) against conventional DWI (conv-DWI) in terms of image quality and lesion detection.

Main Methods:

  • Single-shot EPI-DWI was performed using both L1-regularized iterative SENSE reconstruction (L1-DWI) and conventional parallel imaging reconstruction (conv-DWI).
  • The study included healthy volunteers and a patient cohort (75 subjects) with 349 focal liver lesions (FLLs).
  • Image parameters and FLL conspicuity were rated, and lesion detectability was analyzed.

Main Results:

  • In volunteers, L1-DWI demonstrated significantly higher signal-to-noise ratio and lower standard deviation of apparent diffusion coefficient values compared to conv-DWI.
  • In patients (Group A, reduced acquisition time), FLL conspicuity was rated significantly lower on L1-DWI.
  • Eleven out of 349 FLLs were not detectable on L1-DWI but were visible on conv-DWI in five patients.

Conclusions:

  • L1-regularized iterative reconstruction of single-shot EPI DWI shows potential for accelerated acquisition or improved spatial resolution in liver MRI.
  • The reduced conspicuity and non-detectability of FLLs on L1-DWI in some cases necessitate further research before widespread clinical adoption.
  • Optimization of L1-DWI parameters is required to balance speed/resolution benefits with diagnostic accuracy for focal liver lesions.
