High-Resolution Transperineal Ultrasound in Anorectal Malformations-Can We Replace the Distal Colostogram?

High-resolution transperineal ultrasound accurately diagnoses anorectal malformations (ARM) in newborns, potentially replacing radiation-heavy distal colostograms. This imaging technique offers a safer, faster, and less distressing diagnostic alternative for pediatric surgical conditions.

Background:
- Anorectal malformations (ARM) are congenital defects affecting 1 in 5,000 newborns.
- Diagnosis and classification of ARM, especially without a visible fistula, traditionally require an augmented pressure distal colostogram.
- This colostogram involves radiation exposure after a diverting colostomy.
Observation:
- Four male newborns with ARM and imperforate anus underwent high-resolution transperineal ultrasound with 3D reconstruction.
- The ultrasound assessed the urethra, rectum, and potential fistulas prior to reconstructive surgery.
- Image acquisition was rapid and well-tolerated by the infants.
Findings:
- High-resolution transperineal ultrasound with 3D tomographic reconstruction demonstrated excellent correlation with conventional augmented pressure distal colostograms.
- The ultrasound accurately diagnosed the type of ARM in all four cases.
Implications:
- Transperineal ultrasound may replace distal colostograms in the preoperative assessment of ARM.
- This approach can significantly reduce or eliminate radiation exposure for neonates.
- It offers a faster, non-distressing diagnostic method for a common congenital anomaly.
