Background:

  • The Woven EndoBridge (WEB) device is a novel intrasaccular flow disruptor designed for treating bifurcation aneurysms.
  • Intracranial aneurysms pose significant risks, and treatment strategies are continuously evolving.

Purpose of the Study:

  • To evaluate the efficacy and safety of the WEB device in treating ruptured versus unruptured intracranial aneurysms.
  • To compare aneurysm occlusion rates and complication profiles between ruptured and unruptured aneurysms treated with the WEB device.

Main Methods:

  • A multicenter international cohort study included 676 patients with 691 aneurysms treated with the WEB device.
  • Aneurysms were stratified into ruptured and unruptured groups.
  • Propensity score matching (PSM) was employed to control for baseline characteristics.

Main Results:

  • No significant difference in adequate aneurysm occlusion rates at last follow-up was observed between ruptured (84.3%) and unruptured (85.8%) aneurysms in the unmatched cohort, and (94.4% vs 83.3%) in the matched cohort.
  • Ischemic and hemorrhagic complication rates were similar between the ruptured and unruptured groups.
  • No aneurysm rebleeding was documented post-WEB device implantation.

Conclusions:

  • The Woven EndoBridge (WEB) device demonstrates comparable radiologic outcomes and complication rates for both ruptured and unruptured intracranial aneurysms.
  • Treatment of ruptured aneurysms with the WEB device is feasible and effective.
