    Summary

    Rectal biopsies revealed characteristic inclusions in histiocytes, confirming adrenoleukodystrophy (ALD) in affected individuals. These ultrastructural findings were also present in presymptomatic cases, aiding early diagnosis of ALD.

    Background:

    • Adrenoleukodystrophy (ALD) is a rare genetic disorder affecting the adrenal cortex, brain, and spinal cord.
    • The disease is characterized by the accumulation of very-long-chain fatty acids (VLCFAs).
    • Diagnosis often relies on biochemical assays and neuroimaging, but early detection can be challenging.

    Purpose of the Study:

    • To investigate the presence of ultrastructural abnormalities in rectal mucosal biopsies of patients with ALD.
    • To determine if these abnormalities can aid in the diagnosis of ALD, including presymptomatic cases.

    Main Methods:

    • Rectal mucosal biopsies were obtained from seven individuals across five families with diagnosed ALD.
    • Ultrastructural analysis was performed on the biopsies to identify characteristic cytoplasmic lamellar inclusions.
