    Summary

    Verapamil slightly reduced pulmonary artery pressure in patients with pulmonary hypertension. However, some patients experienced negative inotropic effects and increased pulmonary arteriolar resistance.

    Background:

    • Pulmonary hypertension (PH) is a severe condition characterized by high blood pressure in the pulmonary arteries.
    • Effective pharmacological treatments for PH are limited, necessitating research into novel therapeutic agents.

    Purpose of the Study:

    • To investigate the effects of verapamil, a calcium channel blocker, on hemodynamics in patients with pulmonary hypertension.

    Main Methods:

    • A study involving 12 patients diagnosed with pulmonary hypertension.
    • Hemodynamic parameters including mean pulmonary artery pressure, right ventricular work performance, right atrial pressure, right ventricular end-diastolic pressure, pulmonary arteriolar resistance, cardiac index, and stroke volume were measured.
    • The effects of verapamil administration were assessed.

    Main Results:

    • Verapamil administration resulted in a slight but statistically significant decrease in mean pulmonary artery pressure.
    • A significant decrease in the work performance of the right ventricle was observed.
    • No significant changes were noted in mean right atrial pressure, right ventricular end-diastolic pressure, pulmonary arteriolar resistance, cardiac index, or stroke volume.
    • A wide variability in patient responses was observed.
    • Some patients exhibited a marked negative inotropic effect, leading to an increase in pulmonary arteriolar resistance.

    Conclusions:

    • Verapamil demonstrates a modest beneficial effect on pulmonary artery pressure in some patients with pulmonary hypertension.
    • However, the potential for negative inotropic effects and increased pulmonary vascular resistance in a subset of patients necessitates caution.
    • Further research is warranted to identify patient subgroups who may benefit from verapamil and to understand the mechanisms behind adverse reactions.
