Background:

  • The SARS-CoV-2 pandemic necessitated widespread testing to control viral spread.
  • Antigen tests offered an affordable and rapid solution, expanding healthcare system testing capacities.
  • Emergence of SARS-CoV-2 variants in 2021 required enhanced variant tracking strategies, often excluding antigen test data.

Purpose of the Study:

  • To propose and evaluate the recovery of positive antigen test devices for routine SARS-CoV-2 variant tracking.
  • To assess the suitability of this method compared to standard RT-PCR based variant tracking.

Main Methods:

  • Positive antigen test devices were recovered from April 1, 2021, to January 15, 2022.
  • Recovered devices were analyzed using the established variant tracking protocol.
  • Results were compared with positive samples detected by RT-PCR processed for variant tracking.

Main Results:

  • A total of 21,304 samples were processed: 6,297 from recovered antigen devices and 15,007 from RT-PCR.
  • Only 3.63% of samples were inconclusive overall (1.65% from antigen vs. 4.46% from RT-PCR).
  • The lower inconclusivity rate in the antigen group was statistically significant (p < 0.01).

Conclusions:

  • Recovering positive antigen test devices is a suitable and highly recommended method for variant tracking.
  • This approach significantly improves the ability to obtain an immediate and comprehensive overview of circulating SARS-CoV-2 variants.
  • It represents a valuable measure for enhancing public health surveillance strategies during the pandemic.
