    Background:

    • Erectile dysfunction (ED) is a common condition affecting many men.
    • Venous leakage is a potential physiological cause of ED.

    Purpose of the Study:

    • To investigate venous leakage as a cause of impotence in men.
    • To evaluate the role of papaverine-induced erections in diagnosing venous leakage.
    • To assess the dynamic changes in penile hemodynamics during pharmacologically induced erections.

    Main Methods:

    • The study included 21 men presenting with impotence.
    • Combined pressure flow measurement and cinecavernosography were utilized.
    • Procedures were conducted before and after intracavernosal papaverine injection.

    Main Results:

    • Papaverine injection induced erections and/or higher pressures at lower flow rates in all participants.
    • A significant decrease or disappearance of venous drainage from the corpora was observed in most subjects post-injection.
    • Corpora became larger and retained more contrast material, indicating improved veno-occlusion.

    Conclusions:

    • An active venous obstructive mechanism plays a vital role in achieving and maintaining an erection.
    • Papaverine administration effectively simulates and evaluates this mechanism.
    • This diagnostic approach should be considered in the evaluation of impotent men to identify venous leakage.
