• Extensive diagnostic workups, including EGD, colonoscopy, CT, and ultrasound, were inconclusive.

Findings:

• The patient was diagnosed with a hypersensitivity reaction to titanium clips used during the cholecystectomy.
• A subsequent diagnostic laparoscopy identified and removed four titanium clips, resolving the patient's symptoms.

Implications:

• This case highlights the potential for hypersensitivity reactions to titanium surgical clips, even in patients with a history of inert material tolerance.
