    Background:

    • Sarcoidosis is a multisystem inflammatory disease.
    • Hemoptysis is a potential but infrequent symptom in sarcoidosis.

    Observation:

    • A review of 433 sarcoidosis patients identified hemoptysis in 6% (25 patients).
    • The majority of hemoptysis cases were mild (19 patients), with fewer moderate (4) and massive (2) instances.
    • Clinical, radiographic, and laboratory features were similar between patients with and without hemoptysis, except for higher eye involvement in controls.

    Findings:

    • Hemoptysis in sarcoidosis is generally not associated with distinct clinical or radiographic markers, apart from eye involvement.
    • Bronchoscopy is useful for diagnosing endobronchial sarcoidosis but often fails to identify bleeding sources unless hemoptysis is massive or radiographic abnormalities are localized.
    • Corticosteroid therapy shows potential utility in managing hemoptysis in some sarcoidosis patients.

    Implications:

    • This study clarifies the prevalence and characteristics of hemoptysis in sarcoidosis.
    • It suggests that hemoptysis is typically a mild manifestation and not indicative of specific localized pathology.
    • Findings support the consideration of corticosteroid therapy for symptom control in affected patients.
