    Summary

    Flap coverage effectively treats skin hyperesthesia and neuralgic rest pain. However, this surgical approach does not significantly relieve painful neuromas in continuity.

    Background:

    • Nerve-related pain conditions, including painful neuroma, skin hyperesthesia, and neuralgic rest pain, significantly impact patient quality of life.
    • Surgical interventions like excision, neurolysis, and flap coverage are employed to manage these challenging conditions.

    Purpose of the Study:

    • To evaluate the efficacy of surgical management, specifically flap coverage, for patients experiencing painful neuroma, skin hyperesthesia, or neuralgic rest pain.
    • To determine which of these nerve pain conditions are most responsive to flap coverage interventions.

    Main Methods:

    • A retrospective follow-up study of 14 patients who underwent excision of skin and scar, neurolysis, and subsequent coverage with pedicled or free flaps.
    • Data collected on the improvement or elimination of painful neuroma, skin hyperesthesia, and neuralgic rest pain at a mean follow-up of 20 months.

    Main Results:

    • Painful neuroma showed improvement in 3 out of 7 patients.
    • Skin hyperesthesia was eliminated in 8 of 11 patients and improved in 3.
    • Neuralgic rest pain was eliminated in 5 of 6 patients and partially relieved in one.

    Conclusions:

    • Flap coverage is an effective treatment for eliminating or improving skin hyperesthesia and neuralgic rest pain.
    • Painful neuromas in continuity did not demonstrate significant relief following flap coverage procedures.
