Purpose of the Study:
- To document and maintain SOAP notes for patients visiting community pharmacies for their health concerns.
- To assess the feasibility and impact of integrating SOAP note documentation into routine community pharmacy services.
Main Methods:
- A community-based cross-sectional study was conducted in two pharmacies in Nepal from July to December 2019.
- 400 patients were recruited using simple random sampling; data included subjective complaints (verified by interview and prescription), objective evaluations, assessments, and plans.
- Descriptive analysis was performed using R programming (4.0.3), with drug interactions checked via Medscape.
Main Results:
- Out of 87 participants (aged 42-51 years), 58.8% were female, 52% illiterate, and 89.8% experienced mild polypharmacy.
- Significant medication-related problems were identified, including 130 major (32.7%) and 41 minor (11.4%) drug interactions.
- Follow-up was planned for 56.5% of patients, indicating a need for ongoing medication management.
Conclusions:
- Documenting SOAP notes in community pharmacies during dispensing offers a novel approach to patient care.
- This practice can enhance community pharmacists' professionalism by enabling comprehensive documentation of complex disease and medication profiles.
