Background:

Purpose of the Study:

  • To investigate and quantify the association between testosterone levels and the occurrence of anterior urethral stricture.
  • To determine if hypoandrogenism is an independent risk factor for urethral stricture.

Main Methods:

  • A case-control study was conducted with 149 patients diagnosed with anterior urethral stricture and 67 controls.
  • Blood samples were analyzed for total testosterone, free testosterone, and bioavailable testosterone.
  • Statistical analysis, including adjusted odds ratios (ORs), was performed to assess the association, controlling for confounding factors.

Main Results:

  • Patients with urethral stricture had significantly lower mean total testosterone levels (394 ng/dL vs. 488 ng/dL) compared to controls.
  • The rate of hypoandrogenism (total testosterone < 300 ng/dL) was substantially higher in the urethral stricture group (26% vs. 7.5%).
  • Increased testosterone levels were associated with decreased odds of urethral stricture, while hypoandrogenism showed a strong direct relationship with urethral stricture (adjusted OR 4.01).

Conclusions:

  • The study demonstrates an independent association between hypoandrogenism and anterior urethral stricture.
  • These findings suggest that testosterone deficiency may play a role in the pathogenesis of urethral stricture.
