Summary
Extrarenal Wilms tumor (ERWT) can rarely present in the urinary bladder. This case highlights the importance of considering ERWT in pediatric patients with bladder masses.

Background:
- Wilms tumor (WT) typically arises in the kidney but can occur at extrarenal sites.
- Primary urinary bladder Wilms tumor is an uncommon presentation.
Observation:
- A 4-year-old female presented with micturition difficulties.
- Imaging revealed a bladder mass; biopsy confirmed triphasic extrarenal Wilms tumor (ERWT).
Findings:
- The patient underwent chemotherapy and partial cystectomy for residual tumor.
- Diagnosis of ERWT was confirmed, with the patient remaining well post-treatment.
Implications:
- Primary bladder WT should be considered in the differential diagnosis of pediatric small blue round cell tumors.
- Early and accurate diagnosis from biopsy is crucial for effective management and prognosis.
