Background:

  • The Monocyte-to-HDL Cholesterol Ratio (MHR) is a novel inflammatory marker.
  • Investigating MHR's role in PSD is crucial for understanding stroke recovery.

Purpose of the Study:

  • To determine the relationship between MHR levels and PSD.
  • To assess MHR as a potential predictor of depression after stroke.

Main Methods:

  • Recruited 411 acute ischemic stroke (AIS) patients within 7 days of stroke.
  • Measured MHR at admission and assessed PSD using the Hamilton Depression Scale (HAMD-17) at 3 months.
  • Utilized multivariate logistic regression and ROC curve analysis.

Main Results:

  • 92 patients (22.38%) developed PSD at 3 months.
  • Significantly higher MHR levels were observed in patients with PSD compared to those without (0.81 vs. 0.61).
  • MHR was an independent risk factor for PSD (OR 6.568), with the highest tertile showing increased odds of PSD (OR 5.018). The optimal MHR cut-off for PSD prediction was 0.55 (sensitivity 87%, specificity 68.3%).

Conclusions:

  • Elevated MHR is associated with a higher incidence of PSD.
  • MHR may serve as a valuable inflammatory marker for predicting depression after stroke.
