Amyloid Fibrils03:03

Amyloid Fibrils

9.8K
Amyloid fibrils are aggregates of misfolded proteins.  Under most circumstances, misfolded proteins are either refolded by chaperone proteins or degraded by the proteasome. However, in the case of a mutation or a disease, these proteins can accumulate to form large clusters and often further assemble to form elongated fibers, called fibrils.
Amyloid deposits were observed as early as 1639 in the liver and the spleen.   In 1854, Rudolph Virchow performed iodine staining,...

Area of Science:

  • Rheumatology
  • Hematology
  • Pathology

Background:

  • Multiple myeloma can manifest with diverse extra-osseous complications.
  • Amyloid arthropathy is a rare manifestation of plasma cell dyscrasias.

Observation:

  • A 56-year-old male presented with osteolytic polyarthritis and an atlanto-axial lesion.
  • Initial presentation suggested advanced myeloma with skeletal involvement.

Findings:

  • Histological examination of a salivary gland biopsy revealed amyloid deposits.
  • Congo red staining confirmed the presence of amyloid, establishing the diagnosis of amyloid arthropathy.

Implications:

  • This case underscores the importance of considering amyloid arthropathy in multiple myeloma patients with polyarthritis.
  • Salivary gland biopsy can be a valuable diagnostic tool for amyloid deposition.
  • Early diagnosis of amyloid arthropathy is crucial for appropriate management and patient outcomes.
