Prognosis in node-negative breast cancer.

    Summary

For breast cancer patients with clear lymph nodes, primary tumor volume, grade, invasive lobular carcinoma presence, and intramammary vessel involvement predict long-term survival. Multiple factors indicate a less favorable prognosis.

    Area of Science:

    • Oncology
    • Pathology
    • Medical Statistics

    Background:

    • Accurate prognostic indicators are crucial for tailoring breast cancer treatment.
    • Identifying factors that predict long-term disease-free survival in early-stage breast cancer is essential.

    Purpose of the Study:

    • To evaluate the prognostic utility of clinical and pathological features in breast cancer patients with pathologically node-negative disease.
    • To identify specific pathological characteristics associated with long-term disease-free survival.

    Main Methods:

    • Retrospective analysis of clinical and pathological data from 207 breast cancer patients.
    • Evaluation of features including primary tumor volume, histologic/nuclear grade, tumor histology, and intramammary lymphatic vessel involvement.
    • Correlation of these features with long-term disease-free survival.

    Main Results:

    • No clinical features demonstrated prognostic value.
    • Four pathological features were found to be significant: primary tumor volume, histologic/nuclear grade, presence of invasive lobular carcinoma, and possibly neoplastic cells in intramammary lymphatics.
    • The combination of two or more of these features correlated with a less favorable prognosis, though not in an additive manner.

    Conclusions:

    • Pathological features, not clinical ones, are significant predictors of long-term disease-free survival in node-negative breast cancer.
    • Tumor volume, grade, histology (invasive lobular), and intramammary lymphatic invasion are key prognostic indicators.
    • Prognosis is influenced by the presence of multiple adverse pathological factors.
