Background:

  • B-type natriuretic peptide (BNP) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) are crucial for assessing cardiac conditions.
  • Established pediatric reference intervals for BNP and NT-proBNP are lacking, and their concordance in pediatric patients is poorly understood.

Purpose of the Study:

  • To evaluate the concordance between BNP and NT-proBNP measurements in pediatric patients.
  • To analyze the correlation and agreement of these cardiac biomarkers across different pediatric age groups and cardiac pathologies.

Main Methods:

  • Paired BNP and NT-proBNP testing was conducted on 311 specimens from 175 pediatric patients.
  • Literature-derived pediatric reference intervals were used to assess concordance based on age and cardiac pathology.
  • Deming regression and Spearman correlation analyses were employed to evaluate the relationship between BNP and NT-proBNP.

Main Results:

  • A strong positive Spearman correlation (r=0.91) was observed between BNP and NT-proBNP.
  • Concordance varied by age group, with highest agreement in neonates (0-10 days) and infants (31 days-2 years).
  • While NT-proBNP to BNP ratios differed among cardiac conditions, overall concordance showed no significant differences; discordant serial results were generally not clinically significant.

Conclusions:

  • BNP and NT-proBNP demonstrate positive correlation and moderate concordance in the studied pediatric population.
  • These findings support the combined use of BNP and NT-proBNP for evaluating pediatric cardiac conditions, with consideration for age-specific interpretation.
