Background:

Generalized pustular psoriasis (GPP) flares are poorly understood outside single-center studies.
Electronic health records (EHRs) offer a valuable resource for studying GPP flares in real-world populations.
Understanding GPP flare characteristics is crucial for improving patient management.

Purpose of the Study:

To analyze the characteristics of GPP flares and their treatments.
To compare patients with documented GPP flares to those without flares using US EHR data.
To identify gaps in current GPP flare management.

Main Methods:

Retrospective cohort study using deidentified EHR data from July 2015 to June 2020.
Adult patients with GPP (ICD-10 code L40.1) were identified.
An algorithm was used to detect GPP flare episodes based on diagnosis codes, care setting, and clinical terms.

Main Results:

Of 1535 GPP patients, 271 experienced 513 documented flares.
Patients with flares had higher comorbidity scores and significantly more inpatient and ED visits.
Common flare treatments included topical and oral corticosteroids, opioids, and other systemic agents, with 24% of flares lacking dermatologic treatment around the event.

Conclusions:

GPP flares are associated with substantial comorbidities and healthcare resource utilization.
A significant unmet need exists for effective GPP flare treatments.
Further research and improved therapeutic strategies are needed for GPP management.
