Bilateral vocal cord paralysis after severe blunt head injury--a cause of failed extubation

A case of posterior fossa epidural hematoma causing bilateral vocal cord paralysis (BVCP) is presented. BVCP was responsible for two failed trials of extubation. For this reason and for long-term management, tracheostomy was performed. Laryngeal function returned 40 days after injury, allowing rapid decannulation.
