Background:

Open-angle glaucoma is a leading cause of irreversible blindness.
Effective management of intraocular pressure (IOP) is crucial for preventing glaucomatous optic nerve damage.
Trabeculotomy is a surgical procedure aimed at reducing IOP by improving aqueous humor outflow.

Purpose of the Study:

To evaluate the complications and long-term efficacy of trabeculotomy for open-angle glaucoma.
To assess the rate of intraocular pressure control and the need for additional medication following the procedure.

Main Methods:

Retrospective analysis of 80 trabeculotomy procedures performed between 1971 and 1975.
Follow-up periods ranged from 1.5 to 2.0 years.
Documentation of intraoperative complications and post-operative intraocular pressure control.

Main Results:

The majority of eyes (2/3) achieved IOP control without additional medication.
Intraoperative complications included anterior chamber bleeding, synechias, and mild cataracts.
Twenty-eight percent of patients required pressure-reducing drugs at 1 year, with 11% remaining hypertonic.

Trabeculotomy demonstrates significant potential for long-term intraocular pressure management in open-angle glaucoma.
While complications can occur, the procedure offers a viable surgical option with a substantial success rate.
