Summary

Positive airway pressure (PAP) adherence varies over time. Short-term PAP compliance is linked to OSA severity and age, while long-term adherence depends on oxygen desaturation and symptoms like nasal obstruction.

Area of Science:

  • Sleep Medicine
  • Respiratory Medicine
  • Medical Device Compliance

Background:

  • Positive airway pressure (PAP) therapy is a cornerstone for treating obstructive sleep apnea (OSA).
  • Patient adherence to PAP therapy often declines over time, necessitating an understanding of evolving compliance factors.
  • Identifying predictors of both short-term and long-term PAP compliance is crucial for optimizing treatment outcomes.

Purpose of the Study:

  • To determine the factors influencing short-term (3 months) and long-term (12 months) compliance with PAP therapy in OSA patients.
  • To differentiate between predictors of adherence at different time points during PAP treatment.

Main Methods:

  • A cohort of 187 OSA patients initiating PAP treatment was analyzed.
  • Data collected included demographics, polysomnography (PSG) results, cephalometric measurements, and physical examination findings.
  • Statistical analysis identified predictors for PAP compliance at 3 and 12 months post-initiation.

Main Results:

  • The PAP acceptance rate was 92.5%, with 3-month compliance at 79.1% and 12-month compliance at 51.3%.
  • Short-term compliance was predicted by higher apnea-hypopnea index and older age.
  • Long-term compliance was associated with increased time with oxygen desaturation below 90% (CT90) and baseline symptoms of nasal obstruction and awakening.

Conclusions:

  • OSA severity (apnea-hypopnea index) and patient age are key determinants of initial PAP therapy adherence.
  • Long-term PAP compliance is influenced by physiological measures like CT90 and subjective symptom reporting, including nasal obstruction and sleep-related awakenings.
  • Tailoring interventions based on these distinct short-term and long-term predictors may improve sustained PAP therapy adherence in OSA patients.
