Fractures: Bone Repair

3.6K
Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the...

Area of Science:

  • Orthopedic Surgery
  • Trauma Management

Background:

  • Tibial plateau fractures, particularly Schatzker Types V and VI, represent complex injuries.
  • Effective management is crucial for restoring function and preventing long-term complications.

Purpose of the Study:

  • To evaluate the clinical, radiological, and functional outcomes of treating Schatzker Type V and VI tibial plateau fractures.
  • To assess the efficacy of the Ilizarov ring external fixator with or without minimal internal fixation.

Main Methods:

  • A study of 52 patients with Schatzker Type V and VI tibial plateau fractures treated with Ilizarov external fixation.
  • Functional outcomes were assessed using the American Knee Society (AKS) score.
  • Clinical and radiological union, along with complications, were analyzed.

Main Results:

  • The mean AKS score was comparable for Type V (82.8) and Type VI (80.70) fractures, and for closed versus open fractures (p <0.05).
  • Road traffic accidents were the most common cause (61.53%).
  • 46.15% of fractures were open.

Conclusions:

  • Ilizarov external fixation is a safe, cost-effective, and efficient treatment for complex tibial plateau fractures (Schatzker Types V and VI).
  • The method yields satisfactory functional and clinical outcomes.
