    Background:

    • Reactive hyperemia is the increase in blood flow following a period of ischemia.
    • Generalized scleroderma is a systemic autoimmune disease affecting connective tissues, including blood vessels.

    Purpose of the Study:

    • To investigate reactive hyperemia in the cutaneous tissue of human fingers in normal individuals and patients with generalized scleroderma.
    • To quantify differences in blood flow recovery after induced ischemia between the two groups.

    Main Methods:

    • Ischemia was induced in finger cutaneous tissue for 6 or 12 minutes using a proximal phalanx cuff inflated to 300 mm Hg.
    • Blood flow was measured using the atraumatic 133Xenon wash-out technique.
    • Key parameters calculated included maximum blood flow, excess cumulative blood flow, and repayment (percentage of pre-ischemic flow recovery).

    Main Results:

    • Patients with generalized scleroderma exhibited decreased maximum blood flow compared to normal individuals.
    • Reduced excess cumulative blood flow and repayment were observed in scleroderma patients.
    • These findings indicate an impaired ability to restore normal blood flow after ischemia in affected individuals.

    Conclusions:

    • Generalized scleroderma appears to negatively impact reactive hyperemia in finger cutaneous circulation.
    • Structural changes in blood vessel walls and/or functional alterations in vascular smooth muscle cells may underlie these observed vascular deficits in scleroderma.
