Background:

Purpose of the Study:

  • To examine the relationship between low serum UA concentrations and sarcopenia.
  • To determine if sarcopenia confounds the association between low UA and mortality.

Main Methods:

  • Utilized National Health and Nutrition Examination Survey (NHANES) data (1999-2006).
  • Included participants with dual x-ray absorptiometry (DXA) body composition data and serum UA levels.
  • Employed logistic regression for body composition associations and Cox proportional hazards regression for mortality, with and without body composition adjustment.

Main Results:

  • Low serum UA (<2.5 mg/dL in women, <3.5 mg/dL in men) correlated with lower lean mass (ALMI, ALMIFMI Z scores), underweight BMI, and increased weight loss.
  • The prevalence of low appendicular lean mass index (ALMI) Z scores was significantly higher in the low serum UA group (29%) compared to the normal UA group (16%).
  • Low serum UA was associated with increased mortality (HR 1.61), but this association was attenuated and became non-significant after adjusting for body composition and weight loss (HR 1.30).

Conclusions:

  • Sarcopenia and weight loss are more prevalent in individuals with low serum UA concentrations.
  • Body composition factors appear to mediate the relationship between low serum UA and elevated mortality risk.
