Area of Science:

  • Emergency Medicine
  • Telehealth
  • Medical Simulation

Background:

  • Telemedicine is increasingly utilized in Emergency Medical Services (EMS).
  • Real-time video communication between paramedics and bystanders at emergency scenes is being explored.
  • Assessing patient conditions remotely is a growing area of interest.

Purpose of the Study:

  • To evaluate the utility of real-time video communication between paramedics and bystanders.
  • To determine the impact of video consultations on patient analysis and treatment at emergency scenes.
  • To assess paramedics' performance in simulated emergency scenarios using video.

Main Methods:

  • 44 experienced paramedics participated in a simulation study.
  • Paramedics communicated via Zoom™ with an experimenter presenting simulated emergency scenarios (trauma, cardiac arrest, opiate overdose).
  • Sessions were recorded and analyzed for paramedic interactions, diagnoses, and treatment decisions.

Main Results:

  • Trauma scenarios were assessed quickly, with immediate instructions for bleeding control.
  • Correct initial diagnoses were achieved by most paramedics in cardiac arrest scenarios.
  • Visual clues were sought by over half of paramedics in opiate overdose cases for differential diagnoses.

Conclusions:

  • Direct video communication aids paramedics in diagnosis, treatment instruction, and preparation.
  • This technology can potentially decrease time to diagnosis and lifesaving treatment, improving patient morbidity and mortality.
  • Video clarity impacts diagnostic speed, and the technology may enhance paramedic mental preparedness.
