Summary

Cryptophthalmos, a rare genetic disorder causing eye casing, presents management challenges. Surgical reconstruction offers cosmetic improvement but rarely restores useful vision in Fraser syndrome cases.

Background:

Cryptophthalmos involves partial or complete skin casing of the eyeball.
It is frequently associated with systemic abnormalities, forming Fraser syndrome.
This rare genetic disorder has limited documented cases and complex management.

Observation:

A neonate presented with orbital swelling and a deformed eye.
Examination revealed a skin-covered right orbit and left eye abnormalities including superior symblepharon.
Associated findings included bilateral ear deformities and right renal agenesis.

Findings:

Surgical intervention involved globe and cyst removal on the right.
Left eye surgery included upper eyelid reconstruction and symblepharon release with amniotic membrane transplantation.
This case highlights the challenges in managing cryptophthalmos and Fraser syndrome.

Implications:

Reconstructive surgery can improve cosmetic outcomes for cryptophthalmos.
Achieving useful vision post-surgery remains a significant challenge.
This report is the first from Nepal concerning cryptophthalmos management.
