    Background:

    • Laryngeal tuberculosis (LTB) is typically a secondary complication of pulmonary tuberculosis (PTB).
    • Changing clinical patterns of LTB necessitate updated diagnostic and management strategies.
    • Understanding these shifts is crucial for otolaryngologists managing laryngeal diseases.

    Observation:

    • A review of 15 patients diagnosed with LTB over 20 years at Massachusetts Eye and Ear Infirmary.
    • Patients had a mean age of 56 years with a 2:1 male predominance.
    • Nine patients exhibited minimal pulmonary lesions, and one had a normal chest radiograph.

    Findings:

    • LTB can mimic laryngeal cancer clinically.
    • Patients showed an excellent response to antituberculosis therapy.
    • The infectivity of laryngeal tuberculosis in this cohort was low.

    Implications:

    • Laryngeal lesions, even with minimal pulmonary involvement, should prompt consideration of systemic diseases like tuberculosis.
    • Otolaryngologists must be aware of LTB as a frequent granulomatous lesion of the larynx.
    • Prompt diagnosis and appropriate antituberculosis treatment are vital for favorable outcomes.
