Background:

  • Inconsistent evidence exists regarding the link between chronic kidney disease (CKD) and sleep duration.
  • This study investigates the long-term association between sleep duration and kidney function decline.

Purpose of the Study:

  • To examine the relationship between average nightly sleep duration and the risk of developing kidney function decline.
  • To assess the impact of different sleep durations on composite renal outcomes in a large cohort.

Main Methods:

  • Retrospective, longitudinal cohort study of 82,001 participants in Japan.
  • Participants categorized by CKD risk and self-reported sleep duration (average nightly).
  • Composite renal outcome defined as a ≥40% eGFR reduction or eGFR <15 mL/min/1.73 m².

Main Results:

  • Long (≥8h) and very long (≥8h) sleep durations were associated with increased incidence of composite renal outcomes (OR 1.22, 1.44).
  • Association significant in low CKD risk groups; significant for long sleep in intermediate CKD risk group.
  • Women showed an increased risk (OR 1.14), while men showed a decreased risk (OR 0.91).

Conclusions:

  • Average sleep durations of 8 or more hours per night are linked to increased incidence of poor renal outcomes.
  • Further longitudinal studies are needed to confirm if sleep duration can prevent adverse renal outcomes.
