    Summary

    Screening procedures significantly impact dexamethasone suppression test (DST) results in healthy controls. Many individuals screened as healthy show psychiatric history, and nonsuppressors have higher familial risk for affective disorders.

    Background:

    • Divergent literature on dexamethasone suppression test (DST) nonsuppression rates in healthy controls suggests methodological variance.
    • Screening procedures may explain inconsistencies in DST results among control populations.

    Purpose of the Study:

    • To investigate the impact of screening procedures on DST nonsuppression rates.
    • To examine the psychiatric status and familial risk of affective disorders in DST nonsuppressors.

    Main Methods:

    • Subjects responding to advertisements for healthy individuals underwent a typical screening procedure.
    • Structured interviews assessed Axis I and II disorders.
    • Eligible subjects completed three monthly DSTs.
    • Family studies evaluated affective disorder risk in relatives of nonsuppressors.

    Main Results:

    • A typical screening procedure excluded 69 of 128 subjects.
    • 36% of remaining subjects had historical psychiatric illness.
    • DST results were stable over time.
    • Relatives of DST nonsuppressors showed significantly higher risk for affective disorders (20.5% vs. 8.8%), especially mania and hypomania.

    Conclusions:

    • Screening protocols are critical for accurate DST interpretation in healthy controls.
    • DST nonsuppression may indicate a predisposition to affective disorders, particularly bipolar disorder.
