Attention-Deficit/Hyperactivity Disorder01:30

Attention-Deficit/Hyperactivity Disorder

171
Attention-deficit/hyperactivity disorder (ADHD) is a neurodevelopmental disorder characterized by persistent inattention, hyperactivity, and impulsivity. It affects approximately 5-8% of children globally, with around 60-70% of cases persisting into adulthood. ADHD has significant implications for educational attainment, social interactions, and occupational success.
Diagnostic Criteria and Symptoms
To diagnose ADHD, symptoms must manifest before age 12 and be evident across multiple settings....

Nonlinear Pharmacokinetics: Dependence of Elimination Half-Life and Dose Clearance

283
The elimination half-life and drug clearance of drugs following nonlinear kinetics can vary with dosage. The Michaelis-Menten parameters and drug concentration influence these factors. As the dose increases, the elimination half-life tends to lengthen, resulting in a reduction in clearance and a disproportionately larger area under the curve. The total clearance can be derived from the Michaelis-Menten equation for drugs following a one-compartment model.

Pharmacovigilance01:19

Pharmacovigilance

965
Post-marketing surveillance is a critical component of pharmaceutical regulation, often uncovering unanticipated adverse drug reactions (ADRs) once a drug is widely used over an extended period.
This process, termed pharmacovigilance, aims to detect, evaluate, and minimize harmful effects related to medication use. The data collection for pharmacovigilance depends on spontaneous reporting systems, where healthcare professionals or patients voluntarily report suspected ADRs.
In some cases, there...

Longitudinal Research02:20

Longitudinal Research

12.2K
Sometimes we want to see how people change over time, as in studies of human development and lifespan. When we test the same group of individuals repeatedly over an extended period of time, we are conducting longitudinal research. Longitudinal research is a research design in which data-gathering is administered repeatedly over an extended period of time. For example, we may survey a group of individuals about their dietary habits at age 20, retest them a decade later at age 30, and then again...

Analysis of Population Pharmacokinetic Data01:12

Analysis of Population Pharmacokinetic Data

364
Analysis of population pharmacokinetic data involves studying the behavior of drugs within diverse populations to understand their pharmacokinetic parameters. Traditional pharmacokinetic methods typically involve collecting samples from a few individuals and estimating these parameters. While these methods are commonly used, they have limitations in capturing the variability in drug response among individuals or heterogeneous populations. Population pharmacokinetics is employed to address these...

Kaplan-Meier Approach01:24

Kaplan-Meier Approach

237
The Kaplan-Meier estimator is a non-parametric method used to estimate the survival function from time-to-event data. In medical research, it is frequently employed to measure the proportion of patients surviving for a certain period after treatment. This estimator is fundamental in analyzing time-to-event data, making it indispensable in clinical trials, epidemiological studies, and reliability engineering. By estimating survival probabilities, researchers can evaluate treatment effectiveness,...

Summary

Methylphenidate use in adults is linked to a significantly increased risk of suicide attempts, particularly in women and those aged 18-40. This study highlights the need for careful consideration before prescribing this medication.

Area of Science:

  • Psychiatry
  • Pharmacology
  • Public Health

Background:

  • Methylphenidate is a prescription medication for attention disorders, increasingly used by adults.
  • Concerns exist regarding potential psychiatric risks and mortality associated with its adult use.

Purpose of the Study:

  • To investigate the association between methylphenidate use in adults and the risk of psychiatric events, including depression and suicide attempts.
  • To assess the impact of methylphenidate on overall mortality in adult users.

Main Methods:

  • A population-based matched cohort study design was employed.
  • Utilized data from the Integrated Primary Care Information system in the Netherlands (2.5 million inhabitants).
  • 9,005 adult methylphenidate users were matched with 10 non-users, totaling 97,198 participants.

Main Results:

  • While unadjusted risks for depression and suicide attempts were high, they became non-significant after adjusting for confounding factors.
  • The risk of suicide attempts remained significantly elevated after full adjustment (HR 2.0), especially in women and adults aged 18-40.
  • A significant 30% increased risk of overall mortality was observed (HR 1.3) after full adjustment.

Conclusions:

  • Methylphenidate use in adults is associated with an increased risk of suicide attempts, particularly in younger adults and women.
  • This finding necessitates careful risk-benefit assessment before prescribing methylphenidate to adults, especially those in higher-risk demographics.
